 IMPORTANT NOTES: 

In accordance with the Patient Protection and Affordable Care Act, attached you will find the required Summary of Benefits and Coverage (SBC) for all three medical Plans.  In the event you wish to have a copy and are unable to print the document for yourself, please contact our office and we will provide you a copy free of charge.

Please keep in mind that these are generic summaries of benefits and coverage provided by Blue Cross and Blue Shield; to know specific coverage under the Ave Maria Employee Health Plan, refer to the Benefits-at-a-Glance for your specific Plan located under Benefits, Medical Insurance.

The IRS has announced new maximum contribution limits for 2013:

  • Beginning in January, you can contribute as much as $17,500/year to your retirement plan account.
  • If you are 50 or older, you could contribute as much as $23,000 with the age 50+ catch-up contribution.

Medical Flexible Spending:

Note: Medical Flexible Spending Account costs may be incurred from January 1, 2012 through March 15, 2013 for the Plan Year of 2012. Receipts must be submitted no later than March 31, 2013 for reimbursement.


FOR THE 2013 PLAN YEAR . . . YOUR CLAIMS WILL BE PROCESSED AS FOLLOWS:

  • BLUE CROSS BLUE SHIELD IS RESPONSIBLE FOR PROCESSING YOUR MEDICAL CLAIMS AND PRESCRIPTION DRUG CLAIMS ONLY.  You should present your BCBS ID card when obtaining services.
  • DELTA DENTAL FOR DENTAL CLAIMS. Delta Dental does not distribute ID cards as you are not required to present a card in order to obtain services.
  • VSP FOR VISION CLAIMS.  VSP does not distribute ID cards as you are not required to present a card in order to obtain services.  
  • VERUS HEALTH WILL PROCESS ALL SHORT TERM DISABILITY CLAIMS; FLEXIBLE SPENDING REIMBURSEMENTS; MEDICAL & DEPENDENT CARE AND COBRA ADMINISTRATION

LOOKING FOR BCBS FORMS?  You can find them in the forms directory of this website.  In addition, is you're looking for the Mail Order Drug Forms, logon to: www.bcbsm.com/member/member_forms/index.shtml BE SURE TO VIEW THE BACK OF YOUR BCBS ID CARD FOR THE APPROPRIATE MEDCO CODE SO YOU ACCESS THE CORRECT MAIL ORDER DRUG FORM.

YOU MUST NOTIFY YOUR AVE MARIA HUMAN RESOURCES REPRESENTATIVE WITHIN 30 DAYS OF A LIFE STATUS EVENT; i.e. birth or adoption of a child; marriage; divorce; death of spouse or child; termination of employment of  spouse; commencement of employment of spouse; employee or spouse change from full-time or part-time status or vice versa; or significant change in the Employee’s or spouse’s health coverage attributable to the spouse’s employment.  
 
IT IS THE RESPONSIBILITY OF EACH EMPLOYEE TO MAKE SURE THEIR PERSONAL INFORMATION IS UP-TO-DATE; (i.e. social security number, address, home phone) PLEASE NOTIFY AVE MARIA HUMAN RESOURCES IN THE EVENT OF A CHANGE.

If you participate in direct deposit of your paycheck you may access your biweekly paystub and annual W-2 using the ESS portal.  The new Self Service URL is (https://personal.ecipay.com/Prod/Login).  The client code is AVEMARIA. 

If you do not currently have your check direct deposited, we’d like to encourage you to reconsider; the benefits of Direct Deposit are numerous:
·         Save your employer postal and administrative costs
·         There are no checks to be lost or stolen
·         Peace of mind . . . payments reach your account the day the check is issued – even if you are out of town, sick or unable to get to your financial institution
·         Many banks offer free or lower-cost checking for customers with direct deposit because it saves them the cost of processing paper checks
·         It can save you trips to the bank and help you avoid long lines at tellers or ATMs
·         Direct Deposit is more confidential – 3 to nine people at a financial institution handle each check compared with 1 or 2 people who handle a Direct Deposit
·         There is considerably more security and control with an electronic transaction
 
To sign up for direct deposit today, please complete a Direct Deposit Authorization form and submit to your designated Human Resources Representative.  Please note, it normally takes one payroll cycle for the Direct Deposit to take effect.
