Dental Insurance
There are two Plan design options to choose from provided through Delta Dental plus an opt-out feature. There were no changes to our Dental Plan for 2013.
Enhanced Dental - 2013:
Enhanced Plan pays 100% of reasonable & customary (R&C) charges for cleaning, exams & X-ray twice a year under preventive care. Plus the Plan pays 75% of R&C charges for basic treatment such as fillings, crowns, etc. and 50% of R&C charges for major treatment such as bridges, dentures, etc. Under this plan there is a calendar year deductible of $50/individual; $150/family and an annual maximum of $1,200/per person per year. Includes an orthodontia benefit, the plan pays 50%, not subject to deductible; $1,500/per person lifetime maximum.

*The costs for these services do contribute toward the annual maximum of $1,200/person.
Oral surgery coverage will be provided under the Enhanced Dental option. The Plan will provide 75% coverage for these services with the participant paying 25% after meeting the deductible. Dental Plan Summary, click here.
Core Dental - 2013:
Core Plan pays 100% of reasonable & customary charges (R&C) for cleaning, exams & X-ray twice a year under preventive care. Under this Plan there are no calendar year deductibles or annual maximums to meet.

Waiver Option allows for an employee to opt out of the dental plan when proof of other dental coverage is provided; employee receives a biweekly credit of $2.50.

With Delta Dental there is no need for presentation of an ID card. Just tell your provider you have Delta Dental; they may ask for your Social Security number at the time of your appointment for verification purposes.
