To waive medical coverage, employee must provide Human Resources with proof of other coverage. Waiver benefit for opting out of the medical plan is $15.00/pay period.

  • Medical Care - Annual maximum contribution $2,650 per calendar year for 2018
  • Dependent Care - Annual maximum contribution $5,000 per calendar year

These two documents below provide #1 answers to routine questions asked; #2 an example of a claim incurred and how that claim would be paid in all three examples:

To waive dental coverage, employee must provide Human Resources with proof of other coverage. Waiver benefit for opting out of the dental plan is $2.50/pay period.

  Vision Benefits - VSP

Our Vision Plan is a single-option plan; therefore, you either participate in the Vision Plan or you do not.  As in years' past, there is no opt out waiver paid.

For specific information regarding medical, dental and vision benefits, you will have access to the 2014 Plan Document once it is available.  It will be posted here on our website and the Verus Health website.
